Once CPS receives your personal information, it is posted in a secured database that is accessible to you through use of your password. What Information Does CPS Collect?
CPS has been designed to only disclose your personal information to facilitate your use of the services and for eyecare Professionals to supply a higher quality of eye health care and personalized information to CPS by providing you with newsletters and/or relevant educational information via the Internet. CPS will collect certain information about you and maintain and store it on behalf of the board of directors. The personal information we receive and maintain about you will be used to facilitate your use of the services.
As described in the next section, CPS will not and cannot disclose personally identifiable information about you to any third party


Registration and Contact Information

When you register to use the CPS Services, you will be required to provide certain information, including your name, address, email address, phone number . This information is used to ensure the relevance and appropriateness of information sent to CPS and to provide current contact information.

Communication
CPS communicates to registered eyecare Professionals by providing them with updated eyecare content. It is the discretion of the eyecare Professional whether to disseminate the content to an CPS registered Patient(s). CPS also periodically contacts registered CPS members via telephone or email to obtain feedback on issues affecting CPS.


Sharing of Information
CPS has no access to any of your personal information. CPS does not share, sell, or rent your e-mail addresses to outside third parties. Anonymity is supported wherever possible. Disclosure of confidential information will occur only if required to do so by law. If there is such a requirement, CPS will endeavor to provide affected persons with an opportunity to challenge such disclosure through legal means.


Collection or Use of Aggregate Data

CPS will on occasion assemble aggregate data for use by interested third parties. This information includes non-personally identifiable data collected from individuals or groups of individuals regarding such things as demographic information, preferential information, and system usage information. This information will be processed so that no uniquely identifiable information regarding a particular individual can be interpreted or distributed to an outside party.

Retention of Data
CPS retains data collected from a Patient subscriber indefinitely provided that you are a registered CPS .

Upon registering with CPS, CPS will be asked to provide certain information, such as his/her e-mail address, preferences for certain kinds of information and other forms of demographic details. This specific data will not be shared with third parties, but will be used to provide the Patient with customized information chosen by the Patient’s eyecare Professional based upon the Patient’s selected interests and preferences.

Passive Information Collection/Use of Cookies
As users navigate through a Website, certain information can be passively collected (that is, gathered without your actively providing the information) using various technologies and means, such as Internet Protocol addresses, cookies, Internet tags, and navigational data collection. CPS uses cookies to personalize each individual user’s ‘Meye Homepage’ and to save users time when accessing the Services. A cookie is a small text file containing a unique identification number that is transferred from a Web site to the Patient’s computer hard drive. Usage of a cookie is in no way linked to any personal or personally identifiable health information but is designed to recognize a user’s authentication and personalize the CPS site. Cookies allow us to enhance your experience while using CPS through improved interactivity, customization of information, efficiency and download speeds to the user’s computer. Cookies are considered standard practice for most sites operating on the web. A cookie cannot retrieve any other data from the hard drive, pass on a computer virus or capture a user’s e-mail address. Security
The CPS servers are housed in a well monitored and secure data centre where strict authentication is mandatory to access CPS’s servers and related technology. Moreover, data protection mechanisms, security layers and data encryption measures have been implemented to prevent unauthorized access.

As part of the registration process, your password protected. CPS also encrypts the password within its database. It is important for you to protect his/her password and computer from unauthorized access. When you submit personal information to the secure areas of the CPS web site, we encrypt it using Secure Sockets Layer (“SSL”), a software encryption technology.
